It is not much a spoiler, if you look on the map, with 3 versus 1, in the end the 3 others will fight each other. Ah I hadn't realised that the other two factions were fully allied with Caltex or whatever we're calling it.
In the movie it is made clear that the Western Forces (Caltex) and the Florida Alliance are not working ore coordinating their efforts together, just trying the first to be in Washington, D.C. and capture/kill the president of the Loyalist (United states).

It might be a question of semantics but I would still call it the 2nd as you have 2/3 of the same factions still fighting over the US's territory.

See the Florida Alliance in the most advantages, they have shorter supply lines than the Western Forces (Caltex).

Special Forces belonging to Loyalist (United States) near the ruins of the Palace of Fine Arts, San Francisco, this was in the early days when both California and Texas had seceded from the United States but before they formed the Western Forces (CalTex) together. On the Canadian side i do not think we will see battles as its borders the New People Army controlled territory and Loyalist (United States) territory, the Mexcican might see combat as there would be fighting in Arizona and New Mexico, two states under the control by the Loyalist (United States) and who need to be taken under Western Forces (CalTax) control in order for them to have a connection with each other.

Probably not as it has two long land borders with neighbours which would make for much easier escape from the war. Even if both Canada and Mexico sought to restrict refugees the borders are so long and especially in the north lightly monitored that stopping people crossing would be difficult.
Furthermore where would such boat people go. The nearest suitable states are again Canada and Mexico and its arguably easier and safer to reach them by land. Unless someone tries to get to say Cuba or the Bahamas?
What you might get, albeit on a fairly small scale probably would be flight by air with small commercial jets or possibly in some cases full sized airliners carrying people to safety.
One issue however is, assuming the loyalist government is largely defeated what happens with leading members of it seeking to escape to probably Canada to avoid probably brutal punishment by the winners? Depending on how the situation is would the Canadians turn them back and if they did get through what sort of crisis might develop if the winners demanded their return?

Hawaii and Alaska are Loyalist (United States) states, any member of the government can go there to do a Alpine Fortress (German: Alpenfestung) or Alpine Redoubt last stand.
