Inaccurate. The collocation of the alarm circuit short in the fuse panel, and the defect inside the missile decelerator and booster separation module were not coincident. The report was "doctored" to shift blame from the manufacturers to the airman. The missile was supposed to stand on an insulated stand. Lead contacts from missile check cables were supposed to have ground fault interrupters. Neither was present. Those were not installed. So the HYDRAZINE retro-rocket unit involved was ignited by a spark whose path to this day is unknown. There were two recommendations that were not implemented; a manually extracted fuse procedure, and a revised ground fault interrupter setup. The griping of supposed bureaucrats, who complained they were not consulted; I consider expletive deleted nonsense. They were not there and their input would have interfered with the KISS principle of disaster management mitigation. The air farce was "correct" this one time in its actions and cover up.

If that map of the area affected by a chain reaction is accurate your probably talking of hundred's of thousand's of casualties. As well as a lot of economic damage and of course the US would lose its largest port for quite a while.
