What happens when Pedro II son Afonso, Prince Imperial of Brazil dying at age two from epilepsy he manged to survive, will Pedro II have desire to keep the Brazil empire as it is when the Republican coup happens in 1889 or will a coup never happen at all because Afonso menages to live.

The thing is, you might need to alter Pedro's personality. After his first trip out of the country (to Europe and America, where Isabel acted as Regent), all he wanted to do was travel some more. So, it is possible that Pedro might decide that rather than reigning, he wants to travel the world more/whatever, and he abdicates in Afonso's favor. He was OTL not very interested in having one of his daughters succeed him, thinking that a girl couldn't rule in the turmoil of the current events at the time.
In which case, Brazil's survival as a monarchy depends almost entirely on the personality of Afonso. If he's a military man (which is possible) and well respected by the army, then maybe less of a/no coup than OTL. Pedro is probably gonna be less prone to depression knowing he has a son to pass the empire to.

If he has a son, and said son is going to be the future of the monarchy, Pedro might call the army in as he should've done OTL
