TV Tropes has this about The Patrick Troughton Doctor Who story, "The War Games":
"One scene called for the regulars to hear an explosion and then dive down. Patrick Troughton wanted to know how big the explosion was and insisted on a demonstration — and a huge boulder landed right where they were to be standing. Troughton then said, "That's why I wanted to see what it was like.""
So: Troughton is either overruled or gets cold feet about making an insistence. The boulder kills Patrick Troughton, Frazer Hines, and Wendy Padbury.
What are the rippling effects?

It would be a tragedy for both them and their families and could also cause serious problems for the BBC, with questions asked about its attitude to safety. It might also mean the end of the show, both because of the reboot that would be needed and also a feeling it would be disrespectful of the three actors to continue it with new actors/characters.

stevep, They don't finish a brilliant serial like Shada because of a strike bringing the whole BBC to a halt all because of the freaking "Play School" clock The whole Doctor Who series is suspended and almost cancelled all on the whim of a new controller called Michael Grade with a personal vendetta. But there's an asbestos scare and they carry on business as normal? Well that's the BBC for you.
