Despite the signing of the Belavezha Accords, Soviet President Mikhail Gorbachev chooses not to resign and the Soviet Union continues to exist as a rump entity limited solely to the Kremlin.

How? The Soviets were not in any shape to fight a major war by 1991. In a conventional war they would've probably gotten steamrolled by NATO.

If some hard liners got in control and triggered a conflict they would be losing fairly quickly and hence might be tempted to go nuclear to 'save' the Soviet empire.

So the destruction of the Soviet Union can go two ways, soft as OTL ore hard as you can see in the World War III (Der Dritte Weltkrieg) documentary.
