It goes without saying that this scenario relies on Saddam having WMDs. Doctrine says the US goes tactical nuclear. I'm thinking you'd see between 1-3 B-61s with low kt warheads dropped on the Republican Guard divisions in the Karbala Gap, with the 3rd Mech punching through the hole in Iraqi lines and getting to Baghdad a few days earlier.
Alternatively, if the Iraqi attack is more severe of uses bioweapons instead of chemicals, a B-52 fires a nuke-tipped AGM-86 at a target on the outskirts of Baghdad.

Bush would come under extreme pressure to use nukes - from those who say if you don't you leave us open to others one day using nukes - and those who don't think there is a need to nuke Iraq on miltary/political/moral grounds. I think it depends on the number and spread of casualties. Maybe instead of nukes, Bush uses multiple MOAB attacks.
